Leave of absence (LOA) is a term used to describe a period of time that an employee is to be away from his/her primary job, while maintaining the status of employee.

1st paragraph: State your need to take a leave of absence from the office. Tell the employer the number of days you will not be working here.
You should also mention the reason for your absence, stating only the facts. Do not dig into too much detail.
2nd paragraph: Describe the status of the projects under your responsibility. You could also mention that you have handed over the projects to your colleague, who will handle the same during the period of your absence.
3rd paragraph: This is the place where you end the letter by giving your contact numbers on which you can be contacted if required.
You should also thank the manager for understanding your situation and considering your application.
